coumadin:

3 to 4 days after initiating
most frequently involving areas rich in fatty tissues (breasts, buttocks, hips)
most have protein C deficiency
but only minority of those with protein C deficiency will develop skin necrosis with coumadin
treatment:   vitamin K and heparin

 

heparin:

heparin induced cutaneous necrosis also the result of intravascular thrombosis
localized reactions at injection sites frequent (rarely widespread)
typically 5 to 10 days after initiation
may be the result of induction of platelet aggregation
