coumadin:

•3 to 4 days after initiating
•most frequently involving areas rich in fatty tissues (breasts, buttocks, hips)
•most have protein C deficiency
•but only minority of those with protein C deficiency will develop skin necrosis with coumadin
•treatment:   vitamin K and heparin

 

heparin:

•heparin induced cutaneous necrosis also the result of intravascular thrombosis
•localized reactions at injection sites frequent (rarely widespread)
•typically 5 to 10 days after initiation
•may be the result of induction of platelet aggregation
