endemic in Ohio and Mississippi river valleys
unlike other respiratory dimorphic organisms, Blastomycosis is associated with cutaneous involvement (80%) and may be the presenting manifestation, therefore always consider primary pulmonary infection, even without symptoms
tissue phase (37º) thick walled, broad-based buds (“figure eight” appearance)
(mnemonic = Bs blastomycosis with a single broad-based bud)
