•3-30 days after tick bite

 

epidemiology:

•Lyme disease rare in western U.S. :  the wood fence lizard is what the ticks feed off of there, and the spirochetes die from something in their blood

 

vaccine:

•recombinant outer surface protein
•an ex vivo vaccine:  i.e. organisms are killed outside of host;  works in the gut of the tick (antibodies from host’s blood)

 

diagnosis:

•titers take 3-4 weeks to become positive (too long to wait to treat)
•therefore must use western blot to diagnose

 

treatment:

•doxycycline
•amoxicillin in children (but if question of Ehrlichiosis, then doxycycline is treatment of choice)

 

“erythema migrans-like”

•in South Carolina; have not isolated organism
•lone star tick (amblioma americanum)
•treat for lyme and they get better
