A "classic presentation",  usually at the end of the visit as you are walking out the door ("One more thing doc, my hair is falling out").

 

Physical exam:

frontal thinning? androgenetic alopecia
monk's fringe? trichotillomania
seborrheic dermatitis?  treat it
hirsute or acne? androgenic alopecia

 

History:

(and labs only if the history suggests it will be fruitful)

androgenetic alopecia - family history?
androgenic alopecia - irregular menses, acne, hirsute?
telogen effluvium - recent surgery, trauma, procedure, pregnancy, hospitalization?
medication side effect - examine med list big three: coumadin, retinoids, prednisone
thyroid - weight change? GI symptoms? temperature symptoms? mood?
diet? (fad diet, vegan, eating disorder, alcoholic?)
