Apart from jaundice, both diffuse and circumscribed color changes may occur in chronic liver disease:

 

diffuse color change:

muddy gray color largely due to basal cell melanin
common in primary biliary cirrhosis;  generally mild in other forms of chronic liver disease

 

circumscribed color change:

chloasma hepaticum

 

 

pathogenesis:

? increased melanogenesis or decreased melanin degradation
no increase in MSH
hemochromatosis:  hemosiderin in the skin, and excess melanin
