Nodular Vasculitis

AKA erythema induratum  (sometimes used to describe the subset of patients where lesion is associated with tuberculosis)
pathogenesis:   primary insult appears to be a vasculitis of subcutaneous blood vessels with subsequent ischemic changes producing lipocytic injury and inflammation

 

clinical:

chronic, recurrent, often bilateral
tender, subcutaneous nodules or plaques (like all panniculitis)
predilection for calf area
often ulcerate before healing with an atrophic scar

 

ddx:

erythema nodosum (but e. nodosum has short duration, rapid development, does not ulcerate, and affects chiefly the anterior rather than the posterior calves)

treatment:

if PPD (-), bed rest and systemic steroids
potassium iodide
