Purple Plums

a very useful differential diagnosis - it comes up a lot, but like all other dermal presentations, it is a biopsy diagnosis

 

can divide into six categories or three:

three categories Susan Burgin, MD = vascular vs. "bad" vs. benign adnexal neoplasm
or when you see a purple plum, run through all six categories in your head

 

1.Mets
2.Amelanotic Melanoma
3.Sarcoma DFSP, AFX, leiomyosarcoma
4.Vascular Tumor Kaposi's sarcoma, bacillary angiomatosis, angiosarcoma, lymphangiosarcoma of Stewart Treves, pyogenic granuloma
5.Lymphoma/Leukemia* lymphoma cutis, leukemia cutis
6.Benign adnexal neoplasms: eccrine poroma (palm or sole), cylindroma (face or scalp), clear cell acanthoma (leg)

 

ddx includes "proud flesh" i.e. exuberant granulation tissue; single or multiple friable nodules (which usually is obvious from history of a wound at the site, but not always)

 

 

 

*Lymphoma Cutis - includes B-cell lymphoma and tumor stage MF

 

*Leukemia Cutis -

AML is most common type of leukemia to involve skin and gums
also keep in mind Sweets which although less dermal and more pseudo-vesicular, can look very purple plum-like, it is after all neutrophils just like AML
