clinical:

occurs in healthy full term newborn (a history of perinatal hypothermia, asphyxia, or difficult labor is common)
one or more dusky red nodules which often become fluctuant and discharge an oily liquid
check for hypercalcemia
spontaneous resolution in a few weeks

 

histology:

needle shaped clefts in foamy histiocytes and adipocytes
an inflammatory disease, therefore:  multiple foci of fat necrosis associated with striking granulomatous panniculitis

 

classic ddx - sclerema neonatorum
